1. Passport‑Size Photograph

  • Recent photo with a white background
  • Size: 4.5 cm × 3.5 cm (approx. 200×230 pixels)
  • File size: 20 KB–50 KB
  • No hats, sunglasses, or shadows. Show your full face clearly.

2. Signature

  • Sign in blue or black ink on white paper
  • Scan within 240×240 pixels
  • File size: 30 KB–50 KB
  • Don’t use ALL CAPS for your name (stick to your normal handwriting).

3. Left‑Thumb Impression

  • Use clean white paper and ink your left thumb
  • Scan at same pixel dimensions and size limits as the signature
  • Ensure clarity—no smudges or misprints.

4. Handwritten Declaration

Write the exact text below in your own handwriting (not all caps):

“I, __ (Name of the candidate), hereby declare that all the information submitted by me in the application form is correct, true, and valid. I will present the supporting documents as and when required.”

  • Format: handwritten on white paper
  • Scan in JPG/JPEG format
  • Dimensions and size: same as the signature file
